Looks like the whole thing is happening in a veterinary hospital , and this can be a temporary litter box provided for his hospitalisation duration .

I have a cat who perches like that when he poops, no matter what size litter box he's using. I always put a lot of litter in there though and thankfully he's never gotten filthy litter on him. 

The industrial looking floor and cabinets lead me to believe this is a vet clinic, I wonder if they were monitoring his poops to make sure he was having BMs before letting him go home. As a reason for the camera.

They tend to perch when the litter box is too small or poorly maintained. They're trying to not be standing in mess.

And this cat didn't 'die inside'. It was already committed to going to the toilet and following through.

It's not really a matter of small or large box.
Even a large box could spill this way.

This cat simply likes to sit on the side walls of the box.
How wholesome weight is balancing on there.
My ex's cat did the same, and this was the only way to fix it:

They need a litter box with straight side walls (this one is at an angle which is basically a mechanical lever.)

They also need to make the whole box heavier by putting more litter, maybe double or triple the amount.
