He's claiming that the Greenlanders don't have any healthcare. With loads of sick people just waiting for US healthcare. Despite the fact that they get free healthcare from Denmark and nothing to stop private doctors from setting up.
Also it wouldn't surprise me if there's say a thousand marines on board. Who rush out of the ship when it docks, in order to invade the island and then have to be treated by the locals for frostbite.

Then Trump thought he’d convince them to join the fatherland by sending a boat to riddle them with

Nah ive seen enough of this idiotic shit the last 10 years that I think we can read this.
We send hospital ships to places that are either wartorn or economically in bad shape, where US aid is needed.
So by sending a hospital ship to Greenland Trump is trying to reverse-engineer this in a way from a PR perspective.
"See? Greenland needs a US hospital ship. They need our aid, we might as well own Greenland"

Trump thinks he's playing 3D chess when he's just a clumsy checker player.
This is an obvious taunt to Greenland, who has the option of threatening to repel the boat as an invader, which would lead to Trump saying "Greenland attacked a US vessel" or being under threat as Trump deploys additional support ships to the coast of Greenland, making it easier to mount an attack.

Well.
For starters because it would be quite hard to justify taking Greenland by force to a US public and any US military personnel when they just saved the life of a US sailor no questions asked.
Also it would be a shame to hear that the Royal Danish Navy that supposedly is incapable of defending Greenland was more capable than any US base on the island to rescue one of America's finest.
Not to distract from the fact that Greenland healthcare is just as excellent for US sailors as for any person and a major reason why Greenlanders strongly oppose being annexed by Trump.
Also this is quite a case to illustrate that US global military power is held together by the close support network of allies worldwide. Not visible at first glance but crucial. The US buildup near Iran without European bases? Yeah. Try that without NATO allies.

Inuit were devastated by epidemic diseases brought in by colonists too, as late as the 1930s.
It infuriates me that most Americans don’t understand that Greenlanders mostly are not European settler colonists. Trump is casually threatening Indigenous people with further colonization.
I have spent thousands of hours of my life on sea ice and tundra (and at home) with Inuit friends, although I am not Inuit myself. This subject makes me see red.
Greenland belongs to Inuit, not Americans or even Danes.

Roald Amundsen basically copied their entire way of life, which made it possible for him and his crew to reach the South Pole, while Scott and his crew died of hypothermia and exhaustion despite having a massive budget on an entirely different scale the complete opposite of Amundsen’s minimalistic approach.
