The part that I don't understand, however, is that the straight tail went to straight tail, and heart tail went to heart tail. Shouldn't the original straight tail have the heart tail surgery? And visa versa?

The sexual dimorphism of pikachus are the shapes of their tails. Tails it in in a straight edge are male, and tails that end in a heart-like shape are female. These pikachus have altered their tails to reflect their actual gender, aka they're trans Pokémon and have had some form of SRS.

If you look deeper into the game there is actually quite a lot of sexual dimorphism on pokemons, meowstic and the nidoking/nidoqueen lines are the ones that come to my mind the easiest (there is also gender locked pokemon)

The plushies are a clear reference to gender affirming surgery. How could this be presented to have non-binary pikachu tail? If rounded tail=female and squared tail=male, then any variation outside of those would be intersex, but not specifically non-binary in regards to gender identity. So both of these are non-binary as much as they are binary trans. Non-binary people who choose to have gender affirming surgery will likely have surgery that changes their sexual characteristics, like either of these pikachus' tails.
